The Easter QUESTIONNAIRE
What is stressfull and enjoyable about Easter?
You are being invited to take part in a survey for the above research study. Please take the time to read through the information outlined below before deciding whether you wish to take part.
What is the purpose of this study? We are going to conduct a study over the Easter period, as we want to know what is stressful for people at this time, and what is not, and how this affects people’s well being.
Who is conducting this research? This project is undertaking by Dr. Almuth McDowall, assisted by Mr Rory Niblock-Stuart who is undertaking his work placement at the University of Surrey.
Do I have to take part? Participation in this survey is voluntary. Should you decide to take part, this information sheet is yours to keep and by completing the survey you are giving your informed consent to participate. There are no risks involved in completing the survey, however, should you feel uncomfortable answering any of the questions you are free to withdraw at anytime, and are free to do so without providing a reason why. If you wish to withdraw you can do so by simply not submitting the survey.
What will I have to do for the survey? Should you decide to participate, we need you to complete this survey in its entirety (5 sections). The survey consists of a series of questions around a number of topics and also provides an opportunity to contribute your own comments. This takes no more than 10 minutes to complete. We will also get in touch with you to complete a follow up survey after Easter. If you complete both parts of the survey, you will be entered into a prize draw for £20 in shopping vouchers of your choice.
Will what I say in this survey be kept confidential? All results from the survey will be kept strictly confidential. Your responses to this survey will only be seen Dr. Almuth McDowall and Mr Rory Niblock-Stuart; we are using your email address for the sole purpose of matching people’s responses from before and from after Easter. If you would like to receive a detailed copy of the eventual results, please contact one of us using the details at the end of the survey.
